Daniel J. Fornatto, DDS
Dr. Dan Fornatto is a compassionate, considerate, and quality conscious dentist who uses modern techniques and the latest technology to keep his patients’ teeth and gums healthy. He is gentle and sensitive to the individual needs of his patients. Dr. Fornatto provides safe, comfortable dental care to both children and adults.
Dr Fornatto is a member of several dental professional organizations including the American Dental Academy. He is a perpetual student and enjoys learning the latest techniques. Dr Fornatto takes hundreds of hours of continuing education to better the appearance and health of his patients’ smiles.
Dr. Fornatto grew up in Elmhurst. He attended the University of Iowa for his undergraduate degree and graduated from Northwestern University Dental School in 1991. Dr. Fornatto lives in Elmhurst with his wife. He enjoys playing golf, traveling, and spending time outdoors.

Nicholas Fornatto
Dr. Nick Fornatto grew up in Elmhurst, Illinois before attending the University of Wisconsin-Madison. He fell in love with dentistry in Jamaica doing dental missionary work and worked during his college summers as a dental assistant. He obtained his Doctor of Dental Medicine (DMD) degree from Midwestern University’s College of Dental Medicine in 2025.
Ensuring the comfort of his patients and providing them with the highest quality of care is of the utmost importance to Dr. Nick. He loves to restore patients’ confidence in their smiles, and restore chewing function so they can enjoy their favorite foods. Dr. Nick is a committed lifelong learner of dental medicine. He is always working to advance his skillset through continuing education to deliver the best treatment to his patients.
Dr. Nick lives in Chicago with his wife, who is a veterinarian, and their two orange cats - Kirkland and Soup. In his free time, he enjoys cooking his grandmother’s recipes and recipes he finds online. He also enjoys going out and trying new restaurants, watching football, and spending time with friends.
